Aminophylline.
Theophylline is one of the naturally occurring alkaloids of the xanthine group. Aminophylline is a soluble double salt of theophylline and ethylene diamine and this improves the solubility of the latter by about 20 times. This ensures a more reliable absorption of theophylline from the gastrointestinal tract and more predictable effect. Aminophylline promotes diuresis by its action on the renal tubules which involves an increase in the rate of sodium and chloride excretion; this effect is considered weak. It dilates the bronchioles but, unlike other bronchodilators, it strengthens the force of contraction of the myocardium and the muscles of respiration with a corresponding decrease in the work associated with breathing. It inhibits mast cell degranulation and thus mediator release, increases mucociliary clearance and prevents microvascular leakage. It has a slight stimulant action on the cerebrum.
A bronchodilator in the adjunctive treatment of heart failure, especially where there is concurrent chronic obstructive lung disease in small breeds of dogs; acute pulmonary oedema with bronchospasm; cardiac asthma; chronic bronchitis accompanying mitral disease.
